Survey activity report and temporary operations (form 586)

Overview

Surveyors – use this form to:

make a survey activity report
document non-conformances
permit temporary operations under Exemption 07
communicate these matters to us and the vessel owner.

Submitting the form

When the form is completed:

Provide a copy of the completed form to the owner of the vessel. The owner must keep this form on the vessel when a related exemption is in force.
Submit the completed form to the MARS system with the relevant survey code.
Keep a copy for your records.
