Bunkers convention - Certificate of insurance (COI) (form 350)


Overview

All vessels over 1000 GT arriving at or leaving an Australian port or offshore facility need Bunker certificates.

Oil tankers need Bunker and Civil Liability certificates to carry oil. Oil cargo includes crude, fuel, heavy diesel and lubricating oil.

How to complete this form

Your personal and ownership details MUST be identical on this form, the P&I Club Blue Card and the vessel’s registration on the AGSR to avoid delays in processing your application.
  • Ensure that details on the application form are consistent with the blue card (for example, address details, ownership details and ports of registry).
  • Charter details as well as ownership details should be listed on both the application and the blue card.  
  • A fee of A$70 is payable for the issue of a first certificate or, in the case of certificate renewal, when substantial changes are made. A fee of A$40 is payable on annual renewal of the certificate where existing insurance arrangements continue, or are not substantially changed.
  • Submit this form with a copy of your blue P&I Club Blue Card and your fee payment details (include receipt number if via direct debit, or cheque or credit card details on the form).
