Expected delays due to Tropical Cyclone Alfred

Australia Post has closed multiple post offices across south-east Queensland and northern New South Wales and have temporarily paused delivery services across these areas.  This will cause delays in the dispatch and delivery of AMSA issued documents such as seafarer certificates.

Medical examination report (form 232)

Overview

You can use this form to complete a medical examination report.

Submitting the form

Original copy of this report is to be forwarded by the medical inspector to Sonic HealthPlus Seafarer admin team after the examination is completed.

The medical inspector should retain a copy for record purposes for a period of at least 30 years.

A copy may be given to the applicant for their records if requested.

Lodging from overseas

If you are lodging from overseas, please complete Seafarer form 419, attach all required supporting documents and email your application to the following:

You will need to include payment with your application.
