Notice of change of name, address, or nationality (form 210)


Overview

You can use this form to notify AMSA of a change of name, address, or nationality connected with your ship registration.

Your address should be a residential address—we can also record a post office box as a postal address but this will not be seen on the registration certificate.

You may also need to provide additional information depending on the change required. For example:

  • If your company name has changed, you will need to submit a copy of the ASIC certificate of registration on change of name.
  • If your name has changed, you will need a copy of your marriage certificate or other document showing evidence of the name change.
  • If you change nationality, you will need to show a passport or another immigration document. A change to a person or company's nationality may affect the vessel's eligibility to be registered. Please contact the Shipping Registration Office for further information.

The form needs to be submitted with the original registration certificate, along with any additional documentation for a change of name or nationality to:
