AMSA 537

Application for a unique vessel identifier (form 537)

Overview

This form 537 replaces form 568.

A separate application for a unique vessel identifier is not required if you are applying for one of the following permissions using the relevant form:

However if the vessel does not require one of these permissions and requires a unique vessel identifier, you can use this form to:

  • apply for a unique vessel identifier for a vessel that is not yet constructed or
  • apply for a unique vessel identifier for a vessel that is already constructed.

Note: A unique vessel identifier is issued automatically as part of the vessel permission application process, so you do not need to make a separate application for a unique vessel identifier if you are applying for a permission.
