Certificate of medical fitness for near coastal seafarers (AMSA form 1850)
Overview
This form is for seafarers applying for a near coastal certificate of competency. It lets us know you are medically fit for working at sea. (Note: ‘near coastal’ is also known as ‘domestic’).
Note: any GP can do this medical check, you don't need to go to a specialist.
If you have an international marine qualification and a current AMSA form 303, you can use it instead of AMSA form 1850.

How to complete this form
Print AMSA form 1850
- Fill in the applicant details section on the form.
- Call your GP and ask if they can test your eyesight and colour vision with an Ishihara plate test.
- If they can, book in with your GP only.
- If not, see an optometrist who can, then see your GP.
- Bring this form and proof of ID to the appointment(s). This proof of ID can be a:
- passport
- Australian driver's licence
- other photo ID.

Note: if you only have vision in one eye, you will need to see an ophthalmologist before you see your GP. You will need a report from the ophthalmologist that confirms you can still work safely on board.
Submitting the form
Submit this form with your certificate of competency application form 426.
Lodge both forms at an AMSA-contracted Australia post outlet.
