If you want to suspend a certificate, the suspension will not change the expiry date of the certificate. For example, if your certificate is due to expire in six months and you seek to suspend that certificate for six months, your certificate will be expired when the suspension is lifted.

Attach separate documents if you need to provide additional information.
If you are applying as an organisation, please provide documentation confirming your ABN and/or ACN and business name—for example ASIC extract.
If you are making an application in the name of a trust, please supply a copy of the trust deed in the supporting documents section of the application. We require the deed to confirm the full and correct name of the legal entity that is making the application. If a copy of the trust deed is not submitted at the time of making the application, it may result in delays processing your application.
If you are revoking a certificate of survey, non-survey Exemption 02 approval or Exemption 40 approval due to the sale of the vessel, please note that these certificates or approvals can be transferred to a new certificate holder. Before applying to revoke the certificate and/or approval, you should discuss with the purchaser whether they intend to use the vessel as a domestic commercial vessel. If they are, they may prefer to transfer the certificate into their name by completing an AMSA 1780 form instead of you applying to revoke the permission.
